PRIVATE DUTY NURSING SERVICES
Nursing services are provided to clients based on need and as ordered
by their physician to assist in maintaining a safe and therapeutic
in-home environment. Our in-home nursing services include, but are
not limited to, the following:

>> Med Box Prefills >> Venipuncture >> Foot Care >> Diet
>> Foley Catheter Care >> Skin Care >> Wound Care >> Diabetic Care
>> Injections >> Home Safety
IN HOME PERSONAL CARE
Personal Care Services are performed by our staff of Certified Nursing
Assistants and Personal Support Specialists. Upon admission, a Care &
Comfort representative, along with the client and family, will develop
a Personal Care Plan listing the tasks to be completed at each visit.
Plans are routinely reviewed and updated to fit the client’s changing
needs. Caregivers assist clients with daily activities such as:
>> Bathing >> Personal Care >> Assistance With Dressing >> Meal >> Preparation >> Light Housekeeping Tasks
DEMENTIA CARE
Dementia care addresses the special needs of independent seniors
living with dementia. Our goal is to increase your loved one’s quality of
life by helping with the activities of daily living and keeping them safe
and sound at home.
ALZHEIMER'S CARE
Alzheimer’s care from Lady of Fatima Home Care Services helps
seniors who have received a diagnosis of Alzheimer’s or related
disorder. Memory care providers assist with meal preparation,
dressing and bathing, transportation and other tasks to help your
loved one remain at home.
END OF LIFE CARE
End of life care from Lady of Fatima Home Care Services supports
seniors and their families during the end of life transition. Compassionate caregivers provide assistance to the client as well as support to
family members
PEDIATRIC & ADULT LONG-TERM CARE
In-home long-term care pediatric and adult long-term care provides
block time services to clients based on their needs. Services are
provided by Registered Nurses, Licensed Practical Nurses, Certi ed
Nursing Assistants, and/ or Personal Support Specialists. Care &
Comfort will provide assessments for clients under the age of
twenty-one to determine eligibility for services and/or level of service
needed. An assessment will be completed on request (per client,
family, doctor, etc.) for clients over the age of twenty-one
INITIAL CALL
On your initial call, you will speak to a knowledgeable staff member. After collecting the relevant information, an appointment will be made for a FREE, no-obligation home assessment.
FREE HOME HEALTH ASSESSMENT
The initial in-home assessment focuses on determining the care, safety risks and challenges facing a patient. It is also an opportunity to find out as much as possible about your needs and home health issues.
CARE PLAN
A Registered Nurse or other Licensed Health Care Practitioner working with the patient's physician will make a Personalized Care Plan It is presented, and agreed upon by all parties, before providing services to the patient.

Matching the most suitable health care worker for you and ensuring that you, or your loved one, is put in the best possible hands. Of course if there are ever any concerns about a specific health care worker, we can search for and provide a suitable replacement.
